Распространенное мнение о том, что искусственное вскармливание ребенка исключительно молочной формулой на основе белка коровьего молока (БКМ) определяет возможность сенсибилизации его только к белку коровьего молока в возрасте до 6 месяцев, приводит к ошибочной тактике лечения.</p><p>   Изложение клинического случая. Наблюдался ребенок в возрасте 3 месяцев с дебютом атопического дерматита (АтД) легкой степени (по шкале оценки исследователя IGA 2). С 1,5 месяцев находится на искусственном вскармливании молочной формулой на основе БКМ. Пациент осмотрен по месту жительства, и без проведения обследования произведена замена обычной адаптированной формулы на полный гидролизат и назначен наружный уход за кожей с помощью косметического средства. Ребенок осмотрен аллергологом через 3–4 недели указанной диеты. За этот период родителями описана явно отрицательная динамика, что зафиксировано во время приема: АтД средней степени тяжести с признаками вторичной инфекции, IGA 3 с поражением 50 % поверхности кожи. Были проведены диагностические кожные пробы (прик-тесты) с аллергенами молока, яйца, пшеницы, рыбы. Обнаружена высокая сенсибилизация к яйцу и сомнительная к БКМ. Результаты были уточнены при проведении иммуноферментного анализа: IgE к овомукоиду — 0,78 МЕ/мл (норма &lt; 0,3 МЕ/мл); коровье молоко — 0,01 кЕда/л; казеин — 0,01 кЕда/л (норма &lt; 0,1 кЕда/л). Учитывая наличие признаков вторичного инфицирования АтД, назначена наружная терапия в виде сочетания препаратов «гидрокортизон + натамицин + неомицин» дважды в сутки и отменено применение косметических средств на инфицированные очаги при сохранении ими обработки участков сухой непораженной кожи. Рекомендовано ограничить бытовые контакты с куриными яйцами. Через 2 недели наблюдения — поражение кожи остаточное (IGA 0–1). В рацион питания возвращена негидролизованная молочная формула без последующих признаков обострения заболевания.</p></sec><sec><title>   Заключение</title><p>   Заключение. Данное клиническое наблюдение наглядно иллюстрирует тот факт, что сенсибилизация у детей раннего возраста может начинаться и поддерживаться без орального применения статистически кандидатных пищевых аллергенов. Сенсибилизация в возрасте до 6 месяцев является высоко вероятной при среднетяжелом рецидивирующем АтД. Специфическое аллергологическое обследование возможно проводить при развившемся заболевании в раннем возрасте как с помощью прик-тестов, так и определения специфических IgE. Назначение диеты без обследования недолжно применяться ввиду высоко вероятного необоснованного снижения качества жизни как самого пациента, так и его семьи.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Introduction</title><p>   Introduction. It is believed that if the child ate only a cow’s milk formula (CMF), and сow’s milk allergy is the only possible at the age of &lt;6 months. This approach leads to erroneous treatment tactics.</p><p>   Presentation of the clinical case. A child aged 3 months with the debut of atopic dermatitis (AtD) of light degree (on the scale of the assessment of the researcher IGA 2).The patient was fed on artificial with a CMF from 1,5 months. Infant was examined at the place of residence. The CMF was changed to full hydrolysis CMF and without examination. Additionally, cosmetics were applied to the skin as needed. The child was examined by an allergologist after 3–4 weeks of diet. Negative dynamics were observed. Atopic dermatitis has become moderately severe with secondary infection complications (IGA 3, 50 % affected skin). Prick-tests with allergens of milk, eggs, wheat, fish were carried out. High sensitization to egg and controversial to cow’s milk was found. The results were clarified during enzyme immunoassay: IgE ovomucoid — 0,78 IU/ml (norm &lt;0,3 IU/ml); cow’s milk 0,01 kUA/l; casein — 0,01 kUA/l (norm &lt; 0,1 kUA/l). Considering the infected AtD, “hydrocortisone + natamycin + neomycin” 2 times a day was used in the therapy. Cosmetics on infected foci was canceled, while maintaining the treatment of dry unaffected skin. It is recommended to limit household contacts with eggs. After 2 weeks, the rash is residual (IGA 0–1). The cow’s milk formula was returned to the diet without exacerbations of the skin process.</p></sec><sec><title>   Conclusion</title><p>   Conclusion. The case illustrates that sensitization in children can begin and be maintained without oral ingestion of the allergen. Sensitization at the age of up to 6 months is likely with a moderate-severe recurrent course of dermatitis. A specific allergological examination can be conduct with a developed disease at an early age. Skin tests and enzyme immunoassay are applicable in infants.
